This exercise is an utility of FMEA, a design instrument used to systematically analyze postulated failures, determine actions and verifications. I discover it helpful for a goal retrospective centered on a failure.
Running the exercise:
- Explain the failure underneath evaluation (summarize on high of the canvas)
- Split the canvas into the next 4 areas: evaluation -> analysis -> motion -> verification
- Split the crew into smaller teams (two or three individuals every)
- For every of the four areas, give a couple of minutes for writing notes (smaller group), adopted by a couple of minutes for a bunch dialog
What is the failure? What led to it?
What are the results of this failure?
What is its severity? What is the likelihood it would occur once more?
What are the corrective and preventive actions (so it doesn’t occur once more) for this failure?
Given the listed actions, how can we confirm their end result?
